BY-LAW NO.2007-126
BEING A BY-LAW TO AUTHORIZE THE EXECUTION OF AN
AGREEMENT BETWEEN THE MUNICIPALITY AND MINISTRY
OF HEALTH AND LONG-TERM CARE
WHEREAS the Municipality of Bayham has entered into an Agreement with the Ministry of
Health and Long-Term Care under the"Grow Your Own NP Initiative" for funding support for the
position of Nurse Practitioner;
AND WHEREAS the Municipality of Bayham has entered into an employment agreement with
Kristal Pitter for the provision of Nurse Practitioner services under the said NP Initiative;
AND WHEREAS the Municipality of Bayham has entered into an Agreement with Dr. Larry
Leatherdale for the provision of services as the Collaborative Physician for Kristal Pitter.
AND WHEREAS it is deemed necessary to enter into an Agreement with the Ministry of Health
and Long-Term Care for the provision of funding support respecting such Collaborative Physician
services.
THEREFORE THE COUNCIL OF THE CORPORATION OF THE MUNICIPALITY OF
BAYHAM ENACTS AS FOLLOWS.
1. THAT the execution of an Agreement with the Ministry of Health and Long-Term Care be
and the same is hereby authorized.
2. THAT this by-law shall come into full force and effect upon final passing.
READ A FIRST, SECOND AND THIRD TIME AND FINALLY PASSED this 20th day of
December 2007.
