BY-LAW NO.2007-125
BEING A BY-LAW TO AUTHORIZE THE EXECUTION OF AN
AGREEMENT BETWEEN THE MUNICIPALITY AND DR.LARRY
LEATHERDALE RESPECTING THE PROVISION OF
COLLABORATING PHYSICIAN SERVICES FOR THE
MUNICIPAL NURSE PRACTICIONER.
WHEREAS the Municipality of Bayham has entered into an Agreement with the Ministry of
Health and Long-Term Care under the"Grow Your Own NP Initiative"for funding support for the
position of Nurse Practitioner;
AND WHEREAS the Municipality of Bayham has entered into an employment agreement with
Kristal Pitter for the provision of Nurse Practitioner services under the said NP Initiative;
AND WHEREAS it is deemed necessary to enter into an Agreement with Dr. Larry Leatherdale
for the provision of services as the Collaborative Physician for Kristal Pitter.
THEREFORE THE COUNCIL OF THE CORPORATION OF THE MUNICIPALITY OF
BAYHAM ENACTS AS FOLLOWS.
1. THAT the Mayor and Clerk be and are hereby authorized to execute an Agreement between
Dr. Larry Leatherdale and the Municipality of Bayham for the provision of collaborative
physician services.
2. THAT this by-law shall come into full force and effect upon final passing.
READ A FIRST, SECOND AND THIRD TIME AND FINALLY PASSED this 20TH day of
December 2007.
